- Copy of social security card OR W-2 form
- Proof of address: (Examples are) Utility bills, bank statements, W-2's or lease agreements with your correct mailing address are acceptable forms of proof of address.
- Phone numbers and your email address so we can contact you
- The name of the person who referred you to our service
- Set up fee
We accept personal checks, money orders, credit and/or debit cards or just click on the shopping cart on this website to sign up with a debit/credit card online (click on the tab below the tab that tells you how to sign up). Arrangements may also be made by Zelle, Check by phone, Western Union, MoneyGram or Bank Transfer.
The information we need to get started can be emailed, faxed, mailed or sent via text to us. If you mail your information to us, please make sure the listed items above are included. If you choose to fax or email your information, please make sure you call afterwards so we can verify that we've received your information, and that it's legible. We can also accept your credit/debit card payment by phone for your convenience.
